Project

The Colorado CoIIN state team is implementing a co-management approach designed to improve communication and collaboration among families, primary care, and subspecialty providers to improve equitable access to services for all families. Specifically, the team’s efforts are focused on families who live at a distance from their provider teams. The team is accomplishing these goals by establishing role clarification among providers and systemically utilizing shared plans of care to facilitate communication and increase access for families.

Outcomes

To date, the Colorado team has worked to solidify a family engagement strategy within the Special Care Clinic and has made successful adaptations to their Electronic Medical Record and the MyChart family interface to support their co-management and role clarification efforts.
