Project
The overall goal of Minnesota’s CMC CoIIN Project is to improve the quality of life for the state’s CMC, the wellbeing of their families, and the cost-effectiveness of their care. The team aims to advance the system of comprehensive, family-centered care coordination that supports families of CMC.
Outcomes
The team has taken a multi-faceted approach to improve the system. They are promoting shared plans of care as a tool to increase coordination of care across the child’s providers and family; they are also encouraging families and providers to work as colleagues in the child’s care. Additionally, they are promoting the services and supports families need to successfully care for CMC in the home, while also demonstrating the cost-effectiveness and benefits of comprehensive family-centered care coordination.
