Authorized in 2003 to combat the global impact of HIV, PEPFAR represents the largest commitment by any country to address a single disease. It has provided support to over 100 countries, helping them to mitigate the HIV epidemic.
Comprehensive research by the Analytics Hub and KFF shows that PEPFAR’s impact extends well beyond its core mission of fighting HIV/AIDS, influencing a range of health and socio-economic outcomes in low- and middle-income countries (LMICs). Using rigorous methods, we analyzed data from 157 LMICs—including 90 PEPFAR-supported countries—spanning 1990 to 2018. Through difference-in-differences analysis, we compared outcomes before and after PEPFAR’s launch in 2004, uncovering significant spillover effects on maternal and child health (MCH), immunization rates, all-cause mortality, economic growth, and education.
Our analyses found that PEPFAR-supported countries saw marked improvements in childhood immunization rates for measles, DPT, and Hep B, as well as reductions in child and maternal mortality—without evidence of harm to other health sectors. All-cause mortality was 20% lower in PEPFAR recipient countries than what would have been expected without PEPFAR support, with the strongest effect seen in countries receiving higher PEPFAR funding and participating in the COP process. PEPFAR was also linked to increased GDP per capita growth and fewer children out of school. Together, these findings demonstrate PEPFAR’s broad and sustained development impact, reinforcing the value of continued investment in the program.
